چکیده

The aim of our study was to define the rates of stroke or death, survival, and restenosis after enrolment among patients with intracranial atherosclerotic stenosis events, who underwent PTAS plus medical therapy and medical therapy only. Databases, including PubMed, EMBASE, and Web of Science, were comprehensively searched. RevMan software was used to analyze the outcome measures. Outcome measures included primary endpoints which were stroke or death within 30 days after enrolment, primary endpoint which were stroke or death within one year after enrolment, survival rate within one year after enrolment, and restenosis rate. PTAS plus medical therapy group had a higher rate of stroke or death within 30 days (RR=1.451, 95% CI=(1.011-2.084), P=0.044) and restenosis (RR=2.830, 95% CI=(1.351-5.927), P=0.006). PTAS plus medical therapy group and medical therapy group had similar rates of stroke or death within one year (RR=0.987, 95% CI=(0.634-1.539), P=0.956) and survival within one year (RR=0.877, 95% CI=(0.616-1.248), P=0.466). The present meta-analysis indicated that the outcomes of PTAS plus medical therapy were not obviously better compared to the outcomes of medical therapy on patients with intracranial atherosclerotic stenosis.

Does the Stenting Versus Aggressive Medical Therapy Trial Support Stenting for Subgroups With Intracranial Stenosis?

BACKGROUND AND PURPOSE Although the Stenting Versus Aggressive Medical Therapy for Intracranial Arterial Stenosis (SAMMPRIS) trial showed that medical therapy alone was superior to stenting plus medical therapy for preventing recurrent strokes in patients with symptomatic intracranial stenosis, we determined whether SAMMPRIS supported the use of stenting in any subpopulations of patients with s...

Stenting versus aggressive medical therapy for intracranial arterial stenosis: more harm than good

BACKGROUND Atherosclerotic intracranial arterial stenosis is an important cause of stroke that is increasingly being treated with percutaneous transluminal angioplasty and stenting (PTAS) to prevent recurrent stroke. However, PTAS has not been compared with medical management in a randomized trial. METHODS Objective: To determine whether intracranial stenting (using the Wingspan self-expandin...

Current Status and Future Perspective of Stenting for Symptomatic Intracranial Atherosclerotic Disease: A Meta-Analysis

The aim of this study was to evaluate the safety and effectiveness of percutaneous transluminal angioplasty and stenting (PTAS) for intracranial atherosclerotic disease (ICAD) by conducting a meta-analysis. Two independent observers searched PubMed, EMBASE, and Cochrane Library for relevant studies up to 31 December 2016. A meta-analysis was conducted using Review Manager 5.3. Three studies inv...

Plaque-stabilizing statin therapy prior to percutaneous transluminal angioplasty and stenting.

To the Editor: In a recent issue of the Journal, Soeda et al were able to show that rosuvastatin therapy reduced the lipid core and the size of lipid-rich-coronary plaques,1 adding to the known effect of statin therapy in stabilizing atherosclerotic plaques and reducing plaque vulnerability. When linking their findings regarding coronary plaques to those of a recent clinical trial on intracrani...

Randomized Controlled Trial of Symptomatic Middle Cerebral Artery Stenosis

Intracranial arterial stenosis accounts for 33% to 50% of ischemic strokes in Asian populations. Although antiplatelets have been recognized as the core therapeutic regimen for high-risk patients to prevent the recurrent stroke, the WASID study showed that its beneficial effect for intracranial atherosclerosis (≥70%) is still small, with the 1-year risk of ischemic stroke remaining as high as ≈...
